For over 30 years our purpose is to help provide a better quality of life, allowing people to remain in their home with familiar surroundings. Coordinated Caregiving, within North Carolina Community Alternatives Program for Children (CAP/C) and Adults (CAP/DA) waiver, is a service that provides support to a chosen live-in caregiver to help them learn and maintain skills needed to care for a loved one. Quality Health Care Services offers each level of Coordinated Caregiving - Skilled Nursing acuity, High acuity and Low acuity.
Quality Health Care Services Registered Nurses and Care Coordiantors are committed to working with your current CAP Case Manager to make sure your care needs are met. The intital home visit will allow an assessment and development of a care plan that is individualized and can be revised as needs change. Ongoing education, support, monthly contacts and home visits when needed.
- Must be in good health and 18 years of age or older
- Pass a criminal background and health care registery check
- Must be able to describe or demonstrate practical skills related to the beneficiary health care needs. CPR Certified - Must live in the home and be available to assit with ADLs and other care needs
- Daily stipend payments are considered "difficulty of care" tax free payments
- Inform agency of any changes in medical condition or additional needs
